Fertility Assistance Grant Program
The Footprints Of Hope Project awards fertility assistance grants to qualifying applicants who demonstrate financial need and are actively pursuing treatment.
Current Grant Award: $5,000
Eligibility Requirements
Applicants must:
• Be at least 18 years old
• Reside in Kentucky during the initial grant cycle
• Demonstrate financial need
• Provide a personal story
• Demonstrate treatment readiness through provider documentation
Application Review Criteria
Financial Need – 40%
Personal Story – 30%
Treatment Readiness – 30%
